FULL TIME RN/CASE MANAGER (MALONE)
QUALIFICATIONS & REQUIREMENTS
· Registered Nurse, currently licensed in New York State
· 4-day work week (paid for 5 days)
· Two years of comprehensive nursing experience
· Valid driver’s license and transportation
· Demonstrated skill in nursing practice and ability to respond to the medical and emotional needs of patients and families in varied settings
· Must be physically able to lift, transfer and position patient as needed
· On-Call required on weekends and weekdays as scheduled

FULL TIME or PART TIME LPN
The Hospice Licensed Practical Nurse (LPN) provides compassionate and skilled nursing care to terminally ill patients in their homes, long-term care facilities, or inpatient hospice settings. Under the supervision of a Registered Nurse (RN), the LPN supports the care plan, ensures patient comfort, and promotes quality of life during the end-of-life journey.
Key Responsibilities:
Provide direct patient care including monitoring vital signs, administering medications, managing symptoms, and assisting with daily living activities.
Implement and document nursing interventions as outlined in the plan of care developed by the RN.
Monitor and report changes in the patient’s condition, needs, and responses to treatment to the supervising RN and interdisciplinary team.
Provide emotional support to patients and families, promoting a peaceful and dignified end-of-life experience.
Educate patients and families about medications, symptom management, and what to expect during disease progression.
Assist with coordination of care across disciplines and services including home health aides, social workers, chaplains, and physicians.
Ensure timely and accurate documentation in accordance with hospice policies, procedures, and regulatory requirements.
Maintain compliance with HIPAA regulations, safety standards, and infection control protocols.
Participate in on-call rotation, staff meetings, and continuing education as required.
Qualifications:
Current and active LPN license in the state of practice.
Minimum of one (1) year of LPN experience, preferably in hospice, home health, palliative care, or geriatrics.
Valid driver’s license and reliable transportation (for home visits, if applicable).
Basic Life Support (BLS) certification.
Excellent communication, organizational, and interpersonal skills.
Compassionate, patient-centered approach with strong emotional resilience.
Ability to work independently and collaboratively within an interdisciplinary team.
