Position Summary:

We are seeking a passionate and detail-oriented Development and Communications Coordinator to support our fundraising, donor engagement, and communications efforts. This individual will work closely with our management and clinical teams to execute campaigns, manage donor data, and tell the powerful stories of our patients and families through digital and print channels.

Key Responsibilities:

Fundraising & Development

  • Lead the planning and execution of annual fundraising campaigns, including direct mail, online appeals, and special events.

  • Maintain donor records and generate reports in the donor database

  • Oversee donations and prepare timely acknowledgment letters.

  • Support grant writing and tracking deadlines and reports.

Communications & Marketing

  • Develop and schedule content for email newsletters, website updates, and social media platforms (e.g., Facebook, Instagram, LinkedIn).

  • Draft press releases, blog posts, and impact stories that highlight the mission and work of Hospice of the North Country.

  • Coordinate the production of promotional materials (brochures, annual reports, flyers).

  • Maintain brand consistency across all communications.

 Events & Community Engagement

  • Provide logistical and communications support for fundraising and awareness events.

  • Assist with donor stewardship activities, including recognition programs and small gatherings.

  • Represent the organization at community events as needed.

Qualifications:

  • Bachelor’s degree in communications, nonprofit management, marketing, or related field; or equivalent experience.

  • 1–3 years of experience in nonprofit development or communications preferred.

  • Strong writing, editing, and storytelling skills.

  • Familiarity with donor management systems and Adobe Creative Suite.

  • Proficiency in Microsoft Office and social media platforms.

  • Highly organized, detail-oriented, and able to manage multiple projects.

  • Passion for hospice and end-of-life care is a plus.

Compensation & Benefits:

  • Salary range: $29-$32/hour, commensurate with experience.

FULL TIME RN/CASE MANAGER (MALONE)

QUALIFICATIONS & REQUIREMENTS

·         Registered Nurse, currently licensed in New York State

·         4-day work week (paid for 5 days)

·         Two years of comprehensive nursing experience

·         Valid driver’s license and transportation

·         Demonstrated skill in nursing practice and ability to respond to the medical and emotional needs of patients and families in varied settings

·         Must be physically able to lift, transfer and position patient as needed

·         On-Call required on weekends and weekdays as scheduled

FULL TIME or PART TIME LPN

The Hospice Licensed Practical Nurse (LPN) provides compassionate and skilled nursing care to terminally ill patients in their homes, long-term care facilities, or inpatient hospice settings. Under the supervision of a Registered Nurse (RN), the LPN supports the care plan, ensures patient comfort, and promotes quality of life during the end-of-life journey.

Key Responsibilities:

  • Provide direct patient care including monitoring vital signs, administering medications, managing symptoms, and assisting with daily living activities.

  • Implement and document nursing interventions as outlined in the plan of care developed by the RN.

  • Monitor and report changes in the patient’s condition, needs, and responses to treatment to the supervising RN and interdisciplinary team.

  • Provide emotional support to patients and families, promoting a peaceful and dignified end-of-life experience.

  • Educate patients and families about medications, symptom management, and what to expect during disease progression.

  • Assist with coordination of care across disciplines and services including home health aides, social workers, chaplains, and physicians.

  • Ensure timely and accurate documentation in accordance with hospice policies, procedures, and regulatory requirements.

  • Maintain compliance with HIPAA regulations, safety standards, and infection control protocols.

  • Participate in on-call rotation, staff meetings, and continuing education as required.

Qualifications:

  • Current and active LPN license in the state of practice.

  • Minimum of one (1) year of LPN experience, preferably in hospice, home health, palliative care, or geriatrics.

  • Valid driver’s license and reliable transportation (for home visits, if applicable).

  • Basic Life Support (BLS) certification.

  • Excellent communication, organizational, and interpersonal skills.

  • Compassionate, patient-centered approach with strong emotional resilience.

  • Ability to work independently and collaboratively within an interdisciplinary team.
