Hospice is about caring for those who are facing the challenges at the end-of-life. Those challenges may be physical, emotional, psychosocial, and spiritual. Hospice cares for the whole person and their loved ones, supporting them with skilled nursing, family services, spiritual support, volunteer services and bereavement and grief support.

Hospice of the North Country offers a wide range of services for patients, their families, and caregivers.  We know that each patient’s situation is unique, which is why we offer individualized care for patients with illnesses such as cancer, heart disease, Alzheimer's, or respiratory disease. 

Care can be provided at home, or in a nursing home or other facility. Our services are covered by Medicare, Medicaid, Veterans Benefits, and most insurance plans. Our specialized interdisciplinary team consists of nurses, an LPN, social workers, a chaplain, and volunteers—all trained to help you.

Getting started is sometimes the hardest part. Too often, referrals for hospice care are not made until the patient has uncontrolled symptoms or unmanageable pain. Patients benefit much more from hospice services if referred early, and the family can be better prepared and more able to manage the care process.

We provide assistance finding community resources for patients and families.

Bereavement Services

We provide grief and bereavement support for family and friends for thirteen months after the death of their loved one.

No Referral Needed

You don’t need a doctor’s referral for an assessment. A clinicians can meet with you or a loved one to discuss any concerns and provide answers.
